A standalone incident management system with a public reporting portal, staff triage queue and two mobile apps — replacing a self-hosted tool nobody could upgrade.

The challenge
Incidents arrived by phone, email and word of mouth. Nothing was timestamped, nothing was assigned, and the existing self-hosted tracker was pinned to an ancient PHP version that could not be upgraded or moved.
What we built
We rebuilt the whole thing as a standalone Laravel platform: a public reporting portal, a triage queue with severity-based SLAs, evidence attachments, and an audit trail on every status change.
Two Ionic apps followed — one for the public to report an incident with photos and location, one for staff to work the queue in the field.
The outcome
Every incident now carries a reference, an owner and a clock from the moment it is reported. The platform runs on a current PHP version on infrastructure the client controls.
